Items Needed to Start

For each Borrower, please provide below:

1. Copy of Driver’s License
2. Copy of Social Security Card
3. 2 yrs. W‐2 and/or 1099, most recent
4. 2 yrs. Tax Return, most recent
5. 2 months Bank Statements, complete with all pages. If there is a large deposit other than your income/salary, we will need to see the source
6. 2 months Paystubs. If you are self‐employed or a 1099 Recipient, we need Profit & Loss for Year‐to‐Date.
7. If Refinancing, please send the most recent Mortgage Statement, HOA Statement, Homeowner’s Insurance, Survey
8. Please download and sign the Borrower’s Authorization and Credit Card Authorization forms
